Making Appointments
Services are primarily by appointment. You
can call (312)808-7100 to schedule for an appointment at the Student Health
Center. The receptionist will ask you for some basic information and will
usually be able to schedule an appointment within a short time. For an
urgent problem, please contact Melody Jack at (312)808 7102.
The
following two forms need to be completed by the student visiting Student
Health Center for the first time. Click on them to obtain a copy of each.
- Patient Info Form: Students visiting the Student
Health Center to see a healthcare provider for the first time need
to fill out this form and bring it to the appointment.
- Medical History: Students visiting the
Student Health Center to see a healthcare provider for the first
time need to fill out this form and bring it to the appointment.
For women’s Health exams, please fill out
the following form:

Show Policy:
A “NO SHOW” is defined as missing an
appointment without canceling at least 15 minutes before the scheduled
time.
Effective
1st August 2007, - First “NO SHOW” will be noted.

Second “NO SHOW” will result in a $10 fee!
Please note that due to the high volume of
patients we see on a daily basis, we may not be able to accommodate all
walk-ins. Students who wish to be seen at the student health center are
recommended to call the health center and make an appointment at (312)
808-7100. For emergencies, please call IIT Public Safety at 312- 808 -
6363.
