 Health Care Plans
www.bcbsil.com

The university offers two health care options through BlueCross BlueShield of Illinois (www.bcbsil.com).  Both share the same PPO network.  The Network Plan offers a full range of health care benefits with a $15 office co-payment for each visit to a primary care physician; $30 co-pay for a specialist and no co-pay for annual routine physical exams.  Most other services are paid at 90% with no annual deductible.  The PPO (Preferred Provider Organization) Plan offers traditional health benefits with the freedom to seek care outside the network; you have lower deductibles and coinsurance when you select a participating doctor/hospital from the PPO directory.  A same-sex domestic partner meeting eligibility criteria may be covered.

Copies of certified marriage/birth certificates must be provided to include spouse/dependent children on the plan.  A same-sex domestic partner meeting eligibility criteria may be covered.

The monthly cost of coverage to the employee is based on income as indicated below.  The university will pay the remainder of the cost.
