Policy No. B. 11.00

Date of Issue 8/97

Subject: Workers' Compensation

All IIT employees are covered with respect to work-related accidents and illness under the provisions of the Illinois Workers’ Compensation Law. IIT is self insured and all workers’ compensation payments are paid from university funds. It is imperative that all claims information be processed accurately and on a timely basis.

11.01 An employee who sustains a work-related injury is required to report it immediately to his/her supervisor. Failure to report an accident may result in loss of benefits. See Procedure later in this section.

11.02 The first three (3) days of absence due to an accident are not compensable. These days must be covered by drawing accrued sick leave. Should insufficient sick hours be available, the university will provide pay and debit against future sick leave accruals. Sick leave will not be paid for more than 3 days.

11.03 After return to work, the employee is required to make necessary follow-up physician and/or therapy appointments outside of his/her regularly scheduled working hours. Contact Human Resources for assistance in locating such health care providers.

11.04 After the initial 3 days, the employee is removed from the IIT payroll. Workers’ Compensation covers pay for additional qualifying work time lost. Pay is issued by special check written by IIT’s claims administrator. The amount paid represents two-thirds of average weekly gross wages which is not treated as taxable income.

11.05 All insurance benefits will be maintained for a period of 12 months during a workers’ compensation leave if the employee continues to pay the normal employee contribution on a timely basis each month.

11.06 Retirement contributions on behalf of IIT and the employee cease after the first three days of absence until return to active employment.

11.07 Vacation and sick leave accruals cease until return to work.

11.08 The maximum duration of leave due to workers’ compensation is 12 months. After 12 months, the employee is considered separated from employment and all remaining employee benefits cease. This does not affect continuing eligibility for workers’ compensation payments.

11.09 Union employees are governed by the applicable provisions of the appropriate labor agreement.

PROCEDURES FOR HANDLING

ON-THE-JOB INJURIES

1 Main Campus employees injured on the job Monday through Friday between the hours of 8:30 a.m. and 5:00 p.m. should be sent immediately to Counseling and Health Service (CHS) for evaluation and initial treatment.

Depending upon the nature of the illness or injury, CHS may refer the employee to MercyWorks Occupational Medicine or to an emergency room for further treatment.

Outside these Counseling and Health Service hours on Main Campus, the supervisor should contact Public Safety at 8-6300 to transport the injured employee to MercyWorks for treatment. MercyWorks’ hours are 7:00 a.m. to 10:00 p.m. Monday through Friday. It is located at 2600 South Michigan.

Other campuses refer injuries to other MercyWorks locations, to a private physician or, as necessary, to an emergency room.

a. Downtown Campus may use MercyWorks at 614 W. Monroe (Presidential Tower #3) from 7:00 a.m. to 5:00 p.m. Monday through Friday. Telephone: (312) 902-2489.

b. Moffett Campus may use MercyWorks at 81st and Kean in Justice from 8:00 a.m. to 4:30 p.m. Monday through Friday. Telephone: (708) 594-2550.

2 The supervisor must contact Human Resources immediately to report the injury. Both the supervisor and the employee must complete, within the next two working days a report of injury provided by Human Resources. Upon receipt of this, Human Resources will complete the documentation required by the state and the Occupational Safety and Health Administration and will coordinate follow up.

3 CHS will communicate with Human Resources on the disposition of each case including whether the employee received first aid, was sent home for one or more days, or was referred for further treatment.

4 The injured employee is responsible to immediately contact his/her supervisor to report the outcome of treatment and to report the necessity of any further absence from work.

5 Should the absence endure longer than 3 work days, the supervisor must send a Payroll Authorization to Human Resources to remove the employee from the payroll effective the fourth work day absent. The "comments" section of the P.A. should note, "workers’ compensation". Wages are thereafter issued by special check. Time sheet entries for all days absent beyond 3 days should be coded "420", unpaid absence.

6 If the employee has been treated by an outside physician, he/she must bring a release from the attending physician and present it to the supervisor before beginning work.

7 An employee released for work with restrictions must have his/her case reviewed by Human Resources prior to being scheduled to return to work.
