Subject: Counseling and Health Service The Counseling and Health Service (CHS) office is located on the first floor of Farr Hall. Office hours are from 8:30 a.m. to 5:00 p.m., Monday through Friday. For information or assistance, call extension 8-7100. After regular university hours, emergencies should be handled with the assistance of Public Safety. Call extension 8-6300 for aid in transporting the patient to Mercy Hospital located at 26th and Michigan. Identify the patient as an IIT employee. The CHS provides limited services to faculty and staff. These include:

| a. | Evaluation and initial
treatment for work-related illness or injury. Referral to appropriate sources
of follow up care. See Policy B. 11.00, Workers’
Compensation.

| b. | Initial crisis counseling
with an employee to help in identifying community resources and services
which will aid in coping with and overcoming personal problems. These resources
often are available through the health care plan the employee has selected.
The employee may go to CHS on his/her own with full confidentiality or may
be referred by the supervisor or Human Resources.
