Subject: Long-term Disability Insurance

IIT offers a group long term disability insurance that provides continuation of income in the event that the employee becomes disabled for a period longer than 180 days.

6.01 Eligibility1 (Refer to Policy B. 1.00 for definitions)
Eligible Employees:
Full-time Employees
Full-time Faculty
Senior Research Associates
Research Associates
Part-time employees regularly
 working at least 23.25 hours/week

Not Eligible:
Temporary Employees
Adjunct and Part-time Faculty
Student Employees (includes co-op assignments)
Part-time Employees working
 less than 23.25 hours/week

6.02 Eligibility for Participation

For Faculty and Administrative Officers - TIAA Long Term Disability coverage is available to faculty with the rank of Assistant Professor or above and Administrative Officers.

For All Other Employees Not Covered by the TIAA Plan - UNUM Long-Term Disability coverage is available to faculty below the rank of Assistant Professor and administrative, professional, technical, clerical and union employees.1

6.03 When Coverage Begins

UNUM - All eligible employees have coverage on the first day of the month coinciding with, or following the date of hire. An application must be submitted to Human Resources.

TIAA - All eligible employees have coverage on the first day of the month coinciding with or following six months from the date of hire. The six-month waiting period is waived if the employee can provide proof of participation in a long-term disability plan immediately preceding employment at the university.

6.04 Plan Cost

The university pays one-half of the cost of coverage for the TIAA plan with the employee paying the balance of the cost by payroll deduction. Employees pay the full premium for the UNUM plan by payroll deduction. Premiums are waived if the insured individual applies for benefits and submits proof of total disability.

6.05 Schedule of Benefits

The TIAA plan provides a monthly benefit equal to 60% of base salary up to a maximum monthly benefit of $7,500 and an annuity benefit.

The UNUM plan provides a monthly benefit equal to 50%, 40% or 25% of base salary as elected by the employee to a maximum monthly benefit of $5,000.

6.06 Changing Coverage

Employees may request coverage or changes to existing coverage to the UNUM plan during an annual open enrollment period in December for January coverage. There is no annual open enrollment period for the TIAA plan. Eligible employees requesting coverage in the TIAA plan more than 31 days after the date of hire must complete and submit to TIAA an application and Employee's Statement of Health form.
