Update Personal Information

Official forms to change student information may be obtained in the Office of the Registrar in Room 104 Main Building or they can be downloaded here.

Change of Name

All requested changes of student information must be submitted in writing and accompanied by proper documentation.

To change your name on your academic records with IIT, please submit legal documentation (  i.e..  driver's license, court order ) supporting the change with a signed request form and return the items to the Office of the Registrar:

Change of Address

To change your local, permanent, work, billing, or next-of-kin addresses please go to the Academics Tab in your MyIIT Portal. Simply use the Banner Self-Service channel and navigate to the Personal Information folder, followed by clicking on "Update Addresses and Phones".

Change of Student Identification Number/Social Security Number

To change a Student Identification Number please submit a written and signed request and return it with a copy of your social security card to:
