Request for Review and Approval of Proposal Form (Routing Sheet)
All sponsored research and program proposals must be accompanied by a fully approved Request for Review and Approval of Proposal form (Internal Routing Sheet).
This form must be completed by the principal investigator and signed by the Department Chair and/or Dean at least 5 business days prior to the proposal submission. If there are problems accessing the electronic Routing Sheet, please contact OSRP at osrp@iit.edu (Paper Routing Sheets are no longer accepted).
