The waiver and enrollment deadline is 9/15/2014. Eligible students may waive the Student Health Insurance plan by clicking here beginning 7/15. (Not ALL forms of health insurance are accepted to waive the student plan)

Are you an incoming Fall 2014 student? Your records are due by September 22nd. To obtain the Immunization Verification Form or to view the Immunization Requirements as defined by IL State Law, please visit our Immunization section. A registration hold will be placed on your account if you fail to comply.
Health Insurance cards will no longer be sent by mail. To access your insurance card, click here or you may also contact Aetna to request one to be mailed to you at 800-841-3140.
Late Show Policy
Your reserved time slot is very important to us. Please arrive 20 minutes prior to your appointment to complete initial paperwork. Initial paperwork can be found in our forms section and through your patient portal. We highly recommend you print & complete the appropriate paperwork prior to your appointment. If you arrive late, your appointment may be rescheduled.
No Show Policy
A No Show is defined as missing an appointment without canceling at least 2 hours before the scheduled time.
First No Show will be noted.
Second No Show will be assessed a $10 fee.
Psychiatric No Show
For students who do not cancel their psychiatric appointment at least 24 hours prior to their appointment, a No Show will be marked and a No Show fee will immediately be assessed.
- $50 fee for a missed 20 minute psychiatric session.
- $100 fee for a missed 60 minute initial psychiatric session.

First time vaccines are 100% covered by Aetna Student Health if you have the Student Health Insurance Plan. Otherwise, costs are out-of-pocket and you may file a claim for reimbursement through your insurance company with the payment receipt you receive.

As a student of IIT, you are entitled to a wide variety of programs and services. Student Health and Wellness Center (SHWC) is a primary care facility that is sensitive to diversity. You may make an appointment to see one of our Nurse Practitioners and/or physician as many times per semester as needed, with no office visit charge.
Some services require a minimum fee, but these charges are generally much lower than comparable services provided elsewhere. In order to service our students most efficiently, we ask that you make an appointment to see either the NPs or the physician.
You may also access educational / printed materials provided by the SHWC. These materials are designed to help you make informed decisions regarding your health.
