Making Appointments
Services are primarily by appointment. You can call 312.567.7550 to schedule an appointment at Student Health Services if you are unable to schedule one by using the link below. If you call in to make your appointment, the receptionist will ask you for some basic information and will usually be able to schedule an appointment within a short time.

The following two forms need to be completed by the student visiting Student Health Services for the first time. Click on them to obtain a copy of each.
- Patient Info Sheet: Students visiting Student Health Services to see a healthcare provider for the first time need to fill out this form and bring it to the appointment.
- Medical History: Students visiting Student Health Services to see a healthcare provider for the first time need to fill out this form and bring it to the appointment.

No Show Policy:
A "NO SHOW" is defined as missing an appointment without canceling at least 2 hours before the scheduled time.
Effective August 1, 2007, - First "NO SHOW"” will be noted.
- Second "NO SHOW" will result in a $10 fee!
Please note that due to the high volume of patients we see on a daily basis, we may not be able to accommodate all walk-ins. Students who wish to be seen at Student Health Services are recommended to call and make an appointment at 312.567.7550. For emergencies, please call IIT Public Safety at 312.808.6363.
In case of a medical emergency, patients will be transported to Mercy Hospital, located within blocks of the IIT campus, for further evaluation. For all emergencies after hours, call Public Safety at 312-808-6363 for assistance.
For non-emergency needs after hours or on the weekends, students can visit one of the following urgent care facilities available in the Chicago area. (These clinics are not affiliated with IIT)
