DUPLICATE W-2 REQUESTEach year, W-2's are mailed to each employee's home on or before the January 31st deadline. It is important that if your home addresses changes, you must change your address by logging on to Employee Self-Service (Web For Employees), or by sending written notice of your address change to the Payroll Services Office immediately. Because of the large volume of mail being handled by the US Postal Service during W-2 mailing season, sometimes delivery of your mail to your home might be delayed. If you have not received your W-2 in the mail by mid-February, contact the Payroll Services Office and make a written request for the issuance of a duplicate W-2. The Duplicate W-2 request form is available on the Payroll Services website under the Payroll Forms section. Your written request must include your full name, social security number, year W- 2 being requested and the complete address to which the duplicate W-2 is to be mailed. In the event your original W-2 is returned to Payroll Services by the US Postal Service as 'undeliverable', it will be held by Payroll Services until notice is received from you. Payroll Services processes duplicate W-2 requests during W-2 season once per week on Fridays. Once your duplicate W-2 is ready for pick up, you will receive notice from Payroll that it is ready.
