Students are required to report the following to the International Center:
a. Students are required to submit a copy of their EAD card to the IC upon receipt.
b. Students must use the survey for the first time to report 10 days after the start date listed on the EAD card even if they are not employed.
c. Thereafter, students are required to report the following within 10 days of the event:
i. Change in Name
ii. Change in Address
iii. Employment Status
iv. Interruption of Employment
v. Permanent Departure from the U.S. during OPT; or
vi. Change of Immigration Status.
