The due date to turn in
immunization forms for the Fall semester is September 5th, 2008.

No Show Policy:
A “NO SHOW” is defined as missing an appointment without canceling
at least 15 minutes before the scheduled time.
Effective 1st August
2007, - First “NO SHOW” will be noted.
- Second “NO
SHOW” will result in a $10 fee!

Please note that due to the high volume of
patients we see on a daily basis, we may not be able to accommodate all
walk-ins. Students who wish to be seen at the student health center are
recommended to call the health center and make an appointment at (312)
808-7100. For emergencies, please call IIT Public Safety at 312- 808 -
6363.
