Which students are automatically
enrolled in student health insurance?
All students registered for at least 12
credit hours are automatically enrolled in and billed for student health
insurance. This applies to both Day and Evening Division students.
All full-time and part-time J-1 and F-1
international students are automatically enrolled in student health
insurance.
Intensive Trial Advocacy: If you
are enrolled in either the August or January sessions of Intensive Trial
Advocacy, those credits are added to your fall semester hours. If
enrolling in Intensive Trial Advocacy gives you 12 or more credit hours
for the fall semester, then you be billed for student health insurance.
Incompletes: If you are planning on
making up Incompletes during the semester, those credits will not count
in your credit hours for determining whether you will be automatically
enrolled in the health insurance plan.
Full-time domestic students may waive
health insurance coverage under certain circumstances. (See the following
section on how to waive health insurance for additional
information and links to waiver forms.)

How can I get student health
insurance if I am not automatically enrolled?
Students registered for fewer than 12
credit hours, whether in the Day or Evening Division, are not
automatically enrolled in the Student Health Insurance Plan. These
students can elect to obtain coverage by submitting an enrollment form
before the enrollment deadline. The enrollment deadline is Septemeber 1st
, 2008.
Enrollment requests should be submitted
online.
Aetna Student Health does not accept
applications for enrollment from students. All enrollments must be
processed by IIT.

What type of coverage is provided
under the new health insurance plan for the 2008-09 academic year?
IIT will offer students a single student
health insurance plan that is designed to help control increasing costs
while providing students with one of the most comprehensive student
health plans available. For the 2008-09 academic year, the plan costs
$850 for students. (Additional fees are added for spousal and child coverage.)
Highlights of the new plan include:

Access to
Aetna's national Preferred Provider Network, with over 579,000 health
care professionals;
·
$100,000
lifetime aggregate maximum per condition;
·
Annual
deductible of $100 for care received from Preferred Providers and $300
annual deductible for care received from Non-Preferred Providers;
·
Prescription
drug coverage of 100% up to a $1500 maximum per year after a $10 co-pay
for generic drugs and a $30 co-pay for brand-name drugs;
·
Coverage for birth
control pills and devices;
·
Discounts on
vision and dental care and alternative health care services; and
·
Unlimited
medical evacuation and repatriation.

How do I waive student health
insurance?
Domestic students may waive health insurance
coverage if the student presents evidence of other health insurance
coverage under a plan that provides benefits equal to or greater than the
Student Health Insurance Plan. Students must present evidence of coverage
and complete a waiver form by the waiver deadline.
Waiver forms are available at

waiver_request.php. The deadlines for waiving insurance coverage are provided at
the beginning of each semester. September 1, 2008 for fall semester and
January 6, 2009 for spring.

If I waived student health
insurance last year, do I need to do it again?
If you have previously filled a waiver form,
you need not fill another one, unless your outside insurance carrier
changes. If you have a new insurance carrier, you must notify the IIT
Student Health Center by calling (312) 808-7100 or by sending an email to
student.health@iit.edu .

How can I check to see if I am
enrolled in health insurance?
If you have not already received
confirmation from Aetna Student Health, you can check to see if you are
enrolled by confirming that your account has been charged for insurance.
To check you account online, go to IIT's Web for Students.
You can also call IIT Student Health
Center at (312) 808-7100 to confirm that you have been enrolled in the
health insurance plan.

When will I receive my insurance
card?
For students enrolled in health
insurance for the fall semester, insurance cards should arrive in the
mail by the second week of October or sooner. This card will serve as
your insurance identification for the entire academic year. If you are on
a F1 or J1 visa, it will come to the Student Health Center.
If you are enrolling in the health
insurance plan for the spring semester, your card will be mailed by the
first week of February.

Where can I find a Preferred
Provider/In-Network Provider?
The doctors and other health care
providers who belong to Aetna Student Health’s network are called
Preferred Providers. Using Preferred Providers saves you money because
these medical professionals agree to accept negotiated fees that may be
lower than what non-preferred providers would charge. Your health
insurance only pays benefits for reasonable charges. If a non-preferred
provider charges more than the reasonable charge allowance, you must pay
the difference.
The Preferred Provider network extends
across the country. To find a Preferred Provider, you can use Aetna's
online service at www.aetna.com/docfind.
You can use this service to find out whether a specific doctor belongs to
Aetna's network or to find a Preferred Provider in your area.
In addition, you can call either IIT's
Student Health Center at (312) 808-7100 or Aetna's customer service line
at (800) 841-3140 for assistance in finding a Preferred Provider.

How do I file a claim?
If you are being treated by a Preferred
(In-Network) Provider, your claim will automatically be submitted to
Chickering Claims Administrators by the medical provider.
If you are being treated by a
non-preferred provider, you need to verify how your claim will be filed
by the medical provider. Payment may be required up front. If the
provider does not file the claim directly with Chickering Claims
Administrators, then you must file the claim by submitting an itemized
bill immediately after treatment. Your name, social security number, and
school name should be written clearly on all medical bills. Always retain
copies for your records.
Claim forms can be found on Aetna's
website at http://www.aetnastudenthealth.com.
Click on "Student Connection" and then choose "Find Your
School." You will need IIT's policy number, 724532, to find the
information specific to IIT. Select "Help Center" to locate claim
forms. Please note that there are different forms for visits to the
doctor and for prescription drug reimbursements. If you are having
trouble opening these documents online, you can call Chickering's
customer service line at (800) 841-3140 or stop by the Office of Student
Services in room 310F to get a paper copy.

What should I do if my insurance
card hasn't arrived yet and I need to see a doctor?
If you don’t have an insurance card
yet and need to see a doctor, you have a few options.
First, depending on what is wrong, you can
use the Student Health Center at IIT, where you don't need the insurance
information. However, if you prefer to see your own doctor, you will have
to pay out-of-pocket and get reimbursed from Aetna Student Health. (See
the previous section on how to file a
claim for additional information and links to claim forms.)
Alternatively, some doctors might process
your claim without an insurance card if you explain that you have student
insurance and are waiting for your card. You will need to provide the
doctor with IIT's policy number, 724532, and your ID number, which is
your CWID.
Finally, you can print a temporary
insurance card from the Aetna Navigator site. (See following section
about the Aetna Navigator for
more information.)

How do I obtain coverage for my
dependents?
Dependent enrollment is managed directly
by Aetna Student Health. You can find information and the required forms
at http://www.aetnastudenthealth.com.
Click on "Student Connection" and then choose "Find Your
School." You will need IIT's policy number, 724532, to find the
information specific to IIT. Select "Enrollment Forms" to
locate the General Application for Dependent Enrollment. Unlike Student
Health Insurance, the premiums for dependent coverage are not added to
your tuition bill and must be paid directly to Aetna. The cost for
enrolling a spouse, child, or children is explained on the form. The
deadline for enrolling dependents for the 2008-09 academic year is
September 1, 2008.

What about dental insurance?
For the 2008-09 academic year, IIT is
offering students a dental discount plan called Vital Savings. A
description of the program is available online at http://www.aetnastudenthealth.com.
Click on "Student Connection" and then choose "Find Your
School." You will need IIT's policy number, 724532, to find the
information specific to IIT. Select "Vital Savings" . To
participate in this program for the 2008-09 academic year, you must be
enrolled in IIT's student health insurance plan.

What should I do if I lose my
insurance card?
If you lose your insurance card, you need
to report the loss and apply for a replacement. To request a replacement,
you can call Chickering's customer service line at (800) 841-3140.
You can also visit Aetna's website at www.aetnastudenthealth.com to request a replacement card. Click on
"Student Connection" and then choose "Find Your School."
You will need IIT's policy number, 724532, to find the information
specific to IIT. Select "Help Center" and then fill out the
online card replacement request form.
Alternatively, you can print a temporary
insurance card from the Aetna Navigator site. (See following section
about the Aetna Navigator for more information.)

What is the Aetna Navigator?
All students in enrolled in the Student
Health Insurance Plan will have access to the Aetna Navigator, Aetna’s
new online tool. Insured students can view claims activity, receive a
temporary ID number, request ID cards, research prescription drug prices,
find Preferred Providers, email Aetna customer service, and access
wellness information. You can find the Navigator at www.aetnastudenthealth.com. Click on "Student Connection" and then
choose "Find Your School." You will need IIT's policy number,
724532, to access information specific to IIT. You can then select
"Aetna Navigator." First time users will have to register to
access the system.
