SSA Training Sample Documents
Examples of written documents that are required by the regulation can be download from the link above. Each sprout operation should develop specific documents for its own. But the examples in Microsoft Word files here can be used as a starting point for operation specific documentation. The hard copy of these sample documents can also be found in the Appendix of the SSA Training Manual v2.3.
