All Illinois Tech faculty, staff, and students have the ability to use multiple reporting outlets to report areas of concern, from bias reporting to general campus safety feedback. The method of reporting will depend on the type of incident or feedback being reported. Below outlines the various reporting options available, the general types of incidents associated with each.
Bias Reporting Form
Illinois Tech encourages any member of the Illinois Tech community to submit any bias or hate-related incidents that they observe or experience at Illinois Tech. The Office of Community Standards leads a team of campus administrators who review and take action on any such reports.
Anonymous reports are accepted; however, anonymous reporting does make it more difficult to proceed with an investigation without a named reporter to follow up with.

Care Referral
Illinois Tech acknowledges that during a student's college experience, there are instances where our students will express issues, sometimes within and outside of their concerns. These issues can be academic, overwhelming personal circumstances, disruptions in their education, or illnesses. These stressors can impact a student’s educational experience.
When a student, staff, and/or faculty member is concerned about a student’s well-being, they are encouraged to report via the Care Referral to allow the university to better support the individual.

Community Incident Reporting Form
Incident reports can be submitted by Illinois Tech students, faculty, staff, and/or community members regarding concerns about an Illinois Tech community member, information about a possible violation of the Student Code of Conduct, or a possible case of sexual misconduct.
Incident reporting may be completed using the Community Incident Reporting Form or through the following methods:
- Contact the Department of Public Safety at 312.808.6363. After a report is made, the appropriate departments will be notified.
- Student-related concerns may be reported to the Office of Student Affairs at 312.567.3081 or via email at dos@illinoistech.edu.
- Employee-related concerns may be reported to Human Resources through the appropriate departmental representative. Representative contact information is available at iit.edu/hr/about/people.
- Individuals who have experienced sexual harassment or misconduct may file a report with the Title IX Coordinator, Virginia Foster, at 312.567.5725 or foster@illinoistech.edu.
Ethics Point
Ethics Point is a reporting system designed to enhance communication and empower faculty and staff to promote safety, security, and ethical behavior in the workplace.

Illinois Tech Public Safety Feedback
Illinois Tech Safety leadership treats all reported feedback with the utmost seriousness. It is encouraged that Illinois Tech community members share their experiences, suggestions, and general feedback. Such reports are regarded as opportunities to address issues, implement corrective measures, and enhance the quality of service provided to the university community.
Reports can be submitted via the Public Safety Feedback Survey, knorton1@illinoistech.edu, or at 312.567.3919. These reports will be overseen and managed by the Department of Emergency Management, independent of Public Safety, to ensure neutrality and confidentiality throughout the review process.
